Job Description:
Home Health RN is responsible for traveling to a patients’ home to administer their services and helping patients maintain their independence.

ESSENTIAL DUTIES

• Traveling to patients’ homes to evaluate and assess patients
  medical condition as ordered and report any change
• Provides, coordinates, and directs home nursing care
  according to physician orders and agency policies
• Establish, implement, and revise plan of care and reevaluate
  patient needs
• Travel on average 175-250 miles
• Average 25-35 visits per week

EDUCATION AND/OR EXPERIENCE

• Active/unrestricted RN License
• 2 years of recent home health experience
