Northeast Georgia Medical Center (NGMC) has campuses in Gainesville, Braselton, Winder and Dahlonega with a total of more than 700 beds and more than 1,100 medical staff members representing more than 50 specialties.

NGHS also encompasses Northeast Georgia Physicians Group, a multi-specialty group with more than 65 locations, and The Heart Center of Northeast Georgia Medical Center, a cardiology practice with more than 15 locations.NGHSIn collaboration with the hospital administrators, medical staff and Healthcare professionals, this position coordinates care and services for heart failure patients across their treatment continuum, and provides psychosocial support, lifestyle modification education to the patient and their families.

This is a liaison position requiring clinical experience, self direction, attention to detail and patient follow-up.

The ability to communicate effectively with patients, families, Physicians, staff and the public is an essential component of this position.

In conjunction with Physicians and other Healthcare professionals, the Heart Failure Nurse Navigator is responsible for, but not limited to, facilitating interactions between patients and their Physicians, providing logistical support, securing referrals, and assisting with financial and insurance issues.Current licensure as a RN in the State of GA; Heart Failure Cerfitifcation (CHFN) preferred or certification obtained within 6 mosBSN in Nursing RequiredMinimum of 5 yrs in the practice of professional Nursing in an adult acute care clinical setting, or a minimum of 5 yrs in community health.
