In need of a Nurse Practitioner ASAP for the Dual Special Need Plan team.
This will be a part time position with around 30 hours per week. At this time the role is remote. Laptop will be provided.
Job Summary:
A Nurse Practitioner can be the clinical leader of the member’s ICT when the member’s PCP is unwilling or unable to participate on the ICT. The Nurse Practitioner will work to maximize health and social functioning through an integrated, interdisciplinary approach focusing on the member’s needs and stated desires. The Nurse Practitioner will:
Provide evidence-based treatment and clinical leadership to meet the member’s medical, behavioral, and social/functional needs
Ensure routine medical and behavioral health preventive screenings (for members without a diagnosis for a screening condition)
Adopt care management strategies and clinical interventions aimed at preventing unnecessary hospitalizations, institutional based care, and other higher levels of care for members
Provide and/or integrate primary care and behavioral health services and specialty care as needed
In collaboration with the care manager, arrange for behavioral health specialists and treatment plans for those with higher levels of need
Provide medication reconciliation and medication management, in conjunction with the ICT and/or a Health clinical pharmacist
Actively lead and participate as part of the ICT to manage transitions of care and changes in the member’s needs and ICP
Review the ICP and any future revisions
Ensure members’ needs, including SDOH, are coordinated with community partners
Ensure coordination with Medicaid services, including LTSS
