The overall purpose of theBehavioral Health Care Advocate is to coordinate care with members in order to improvethe member’s ability to remain stable in the community and out of the hospital.This coordination includes intensive coordination, discharge coordination, andcare coordination for members.

The Behavioral Health Care Advocate role is toengage the person in the treatment process and then assist him/her to accessthe appropriate community services so they can pursue their treatment goals inthe community and avoid institutionalization.

This position focuses on memberswith high needs, high utilization, and in need of complex Behavioral HealthCare Coordination and works closely with members, providers, Statestakeholders, Utilization Management, and Peer Support.

This position is locatedonsite in our Atlanta, GA office.

Theschedule is Monday Friday during normal business, with one late day a weekuntil 7pm.PrimaryResponsibilities:Performing care management activities to ensure that members move through the continuum of care efficiently and safely Utilize advanced clinical skills to engage and motivate members via a recovery, health and wellness-oriented approach telephonicallyAssist members and their families/caregivers with connections to appropriate psychiatric, medical, and psychosocial referrals and servicesIdentify and remove barriers to procurement, delivery, participation in and success of servicesAssessing and interpreting customer needs and requirementsReviews cases and analyzes clinical information in conjunction with Medical Directors to determine the appropriateness of hospitalization Escalates cases to the Medical Director for case discussion or peer-to-peer intervention as appropriate Performs anticipatory discharge planning in accordance with the patient’s benefits and available alternative resources Collaborates with Medical peers on consideration of discharge planning needsRefers members to disease management or case management programs Assists with the development of treatment plans Documents activities according to established standardsYou’ll be rewarded and recognizedfor your performance in an environment that will challenge you and give youclear direction on what it takes to succeed in your role as well as providedevelopment for other roles you may be interested in.

RequiredQualifications:Licensed Master’s Degree-Level Clinician in Psychology, Social Work, Counseling or Marriage or Family Counseling OR a Licensed Ph.D.

level Psychologist, OR an RN with behavioral health experienceLicense must be active and unrestricted to practice at the independent level in Georgia2 years of experience in a behavioral health setting post independent licensureProficiency with Microsoft Office Suite programs (Word, Outlook, Internet)Ability to work onsite in our Georgia office on a daily basisComputer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applicationsEmployees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.E., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe.

Employees must comply with any state and local masking orders.
