Care Coordinator

This is a Contract position in Atlanta, GA posted December 14, 2021.

Join the A-Line Staffing growing team as a Care Coordinator working for a major healthcare institution in Michigan Apply to be considered immediately Growth Opportunities Remote but need to be in the state of MI Schedule : 8am-4:30pm EST.

Overtime not required Summary
• Monitor and coordinate members’ treatment plans, guide them to understand the status of their health condition, connect them with health care providers, and check in on their members’ progress periodically.

• Collaborate with care team and other members of the team to ensure members goals and needs are met.

• Educate members and responsible parties about health conditions, medications and develop care plans to ensure member’s safety.

• Complete health risk assessments and care plans for members
• Use computer software to coordinate members’ care with all necessary care providers to ensure member needs are met.

• Attend weekly huddles and monthly department meetings to stay up to date on all business initiatives related to the health plan.

• Provide support to medical, behavioral and social support staff to determine needs, eligibility and services for members.

Conduct screening and risk assessment interventions per program guidelines.

Job Requirements: Required Effective verbal and written communication skills Excellent skills/knowledge of Computer Software, Healthcare Management Systems, and Microsoft Office Programs Strong time management and customer service skills Bachelor’s degree in Social Work, Health Services, Behavioral Science, or related field, or equivalent experience.

Preferred Highly organized with a strong ability to manage different priorities Team player and able to work with others Ability to exercise empathy and compassion when working with members and responsible parties LPN, LVN or LCSW preferred
